Consent & disclosures

Please read carefully before agreeing. By submitting this form, you (the family contact) confirm, on behalf of yourself and your parent, that you have read, understand, and agree to each of the following:

What Dote is. Dote is a companionship and check-in service. It is not a medical, mental-health, emergency, monitoring, caregiving, or medical- alert service, is not a medical device, and is not a substitute for professional care or emergency services. Do not rely on it for any health, safety, or emergency need.
Emergencies and limits. If a concern arises, Dote may attempt to contact your parent's emergency contact and direct them to 911, but Dote cannot detect, prevent, or respond to every situation and does not guarantee any monitoring, response, or outcome. In an emergency, call 911.
Automated calls & texts. You and your parent consent to receive recurring automated, prerecorded, and AI-generated phone calls and text messages from Dote at the numbers provided. This consent is not a condition of any purchase. Message and data rates may apply; reply STOP to opt out or HELP for help.
Call recording. Calls may be monitored, recorded, and transcribed to provide and improve the service. You confirm that both you and your parent consent to this recording. Some states require every party on a call to consent to being recorded, and you confirm that consent has been given.
Your parent's consent & your authority. You confirm your parent is aware of and agrees to these calls and to Dote sharing call summaries and any safety concerns with you, and that you are at least 18 and are authorized to enroll your parent and provide these consents on their behalf.
Health information & privacy. Dote is not a HIPAA-covered entity. Information you or your parent share (which may include health-related details) and call summaries will be stored, processed, and shared with the family contact as described in our Privacy Policy. Residents of some states (for example, California) may have additional privacy rights.
No warranties; limitation of liability. The service is provided "as is," without warranties, to the fullest extent permitted by law. This is an early pilot; features may change, and either you or your parent may cancel at any time.
Agreement to terms. You agree to Dote's Terms of Service and Privacy Policy.
